Improved productivity – and quality of life – begins with identifying risks. Our specialized health risk assessment (HRA) takes your staff through a series of questions designed to evaluate current health, health history, and potential health risks. The questionnaire is available online or in paper format, so employees can access it through work or at home. Our nurses also conduct onsite screenings for blood pressure, cholesterol and other health indicators.

A personalized report based on the questionnaire and screening results is created for each employee. The report alerts them to potential health risks and is a strong motivator to help them consider minor or major lifestyle changes and, if needed, treatment for high blood pressure, high cholesterol or other health issues.

Each individual’s report is strictly confidential. However, the Workforce Health Program has access to aggregate HRA data that is used to provide a profile of your organization’s overall health. This executive summary can be combined with your claims review information to become a powerful tool for determining the scope of your employee wellness program.

When the HRA is combined with follow-up services, such as screenings or educational initiatives, you’ll have measurable results that can help individuals change at-risk behaviors and improve their quality of life.
